DISABILITY INSURANCE
Can apply for up to $1,155.00 per month after a thirty day waiting period covers up to two years for a sickness or up to five years on an accident. Also has coverage for over night stays in the hospital and has accidental death benefits of your monthly disability amount for 24 months. This program does coordinate with State Disability Insurance, Worker's Compensation and other group disability insurance. See Brochure for further details.
TERM LIFE INSURANCE
Minimum $10,000.00 life with an additional $1,000.00 per month for 12 months upon an accidental death or up to $250,000.00 life/$300,000.00 additional accidental death. Spouse coverage is also available.
ACCIDENTAL DEATH AND DISMEMBERMENT INSURANCE
Up to $250,000.00 for you and your family.
ACCIDENTAL DISABILITY INCOME
Insurance that pays benefits weekly and pays cash benefits in hospital for accidents.
